This website documents an Unreal Tournament modification project. I think this website shows the process of designing a game very well. It shows the process by which the game designer developed his concept with sketchs of characters, environments, weapons, as well as written desciptions of the characters, the story and the game play. Unfortunately I could not find the game itself.
Take a look at the Game Design Outline at the bottom of this page: http://j-brown.net/alterius/downloads.shtml. I think it will give all of you a good idea of the inital stages of designing a game.
